Terms & Conditions of Participation in Group Music Therapy
*Sound Starts Music provides services in accordance with the Standards of Clinical Practice and Code of Ethics outlined by the American Music Therapy Association (AMTA). These materials may be found online at www.musictherapy.org. The music therapist has received the credentials MT-BC for the Certification Board for Music Therapists. This certification ensures the highest level of professionalism and quality of service for music therapists in the United States.
*Sound Starts Music protects the privacy of all clients and keeps confidential the information obtained in the course of therapy. Confidential information may be revealed only under the following circumstances:
- When, under careful deliberation, it is decided that society, the client, or other individuals appear to be in imminent dangers. In this situation, information may be shared only with the appropriate authorities or professionals..
- When the client/guardian consents to the release of confidential information: In group music therapy, guardian and child(ren) will be interacting with other guardians and children. Sound Starts Music will not be held liable for information exchanged between participants.
*I give consent for Sound Starts Music to provide my child with music therapy services. I consent to care and treatment falling under the practice of the American Music Therapy Association. I acknowledge that there is always a risk of injury with any therapy involving physical activities.
*I understand that this music therapy group is designed to encourage peer and family interaction, and that I (or appointed guardian) must attend each session with my child(ren). In the unlikely event of injury on the premises, I release Sound Starts Music and designated group location of liability.
*I understand that music therapy is generally considered a safe treatment modality for children diagnosed with Autism Spectrum Disorders and other special needs. I give my permission for guardian & child(ren) to participate, but acknowledge that I can discontinue participation at any time, for any reason.
*I understand that registration for this group is not considered complete until payment in full has been made. My child’s spot will not be held until payment is completed.
*I understand that prorated pricing is available only for sessions noted in the registration from prior to first class. Refunds are not available for unexpected missed classes (illness, last minute trip, or decision to discontinue services, etc.) I will still receive each weeks songs and materials regardless of attendance.
*I understand that photos and/or video may be obtained during the course of sessions. These photos will be used for future promotional and educational purposes only. I can request that photo/video of my child not be used at any time via conversation with music therapist.
